New Patient Registration
- Register by completing a registration form which can be downloaded below or obtained from the practice.
- Please bring the completed form along to reception with some proofs of identification (passport, driving licence, etc) and your medical card if you have it.
- When you register, we will ask you to make an appointment to see the Health Care Assistant for a new patient health check.
- Once you have been accepted as a new patient your medical records will be transferred to us and a new medical card will be sent to your home address.
New Patient Checks
As part of the registration process you will also be required to complete a health questionnaire, and to attend the surgery for a new patient check. These checks are designed to ensure that our records are accurate and all routine checks to promote your health and wellbeing (such as blood pressure/smear tests/mammograms) are up-to-date.
