New Patient Registrationregister_now

  1. Register by completing a registration form which can be downloaded below or obtained from the practice.

  2. Please bring the completed form along to reception with some proofs of identification (passport, driving licence, etc) and your medical card if you have it.
  3. When you register, we will ask you to make an appointment to see the Health Care Assistant for a new patient health check.
  4. Once you have been accepted as a new patient your medical records will be transferred to us and a new medical card will be sent to your home address.

New Patient Checks

As part of the registration process you will also be required to complete a health questionnaire, and to attend the surgery for a new patient check. These checks are designed to ensure that our records are accurate and all routine checks to promote your health and wellbeing (such as blood pressure/smear tests/mammograms) are up-to-date.
